  • What are Ms. Cusack's office hours?

    Ms. Cusack holds office hours Monday through Thursday. Morning, afternoon and evening appointments are available.


  • What are Ms. Cusack's fees?

    Ms. Cusack's full fee is $120 a session. Ms. Cusack offers a limited number of sliding fee appointments based on need.


  • What forms of payment are accepted?

    Ms. Cusack accepts cash or check payments. Ms. Cusack asks that you pay for your therapy at the time of service.


  • Does Ms. Cusack accept insurance?

    Ms. Cusack is concerned that participating in managed care plans compromises the privacy and integrity of her client's treatment. Therefore, she does not participate in any managed care plans. If you have a PPO insurance plan, you may be eligible for reimbursement for some or all of the cost of therapy. Ms. Cusack will provide you a statement of services on a monthly basis which you can submit to your insurance company for reimbursement for services from an "out of network provider". It is recommended that you contact the insurance company prior to your first appointment to find out about your plan's coverage for psychotherapy. You can do this by calling the customer service number on the back of your insurance card and asking about your "outpatient mental health coverage". It is important to learn about your out of network coverage including: copayment or co-insurance, your deductible amount, whether preauthorization or certification is necessary, how many visits are allowed, and if there are any restrictions on your policy.


  • What can I expect from the first session?

    The purpose of the first session is for you and Ms. Cusack to become acquainted with one another and determine goodness of fit. The session will last approximately 50 minutes. Ms. Cusack will ask you questions about your current difficulties and some of your history. Feel free to ask Ms. Cusack any questions you may have about her style or approach. It may also be helpful to think about what you would like to accomplish in therapy. At the end of the session, Ms. Cusack will make a recommendation of the type and frequency of treatment that is best for you. You will also discuss your initial goals, though these can also be clarified as treatment progresses.
